Why can't I get these tests through my PCP or cardiologist? About 3BB
Many people simply can't get access to advanced lab and imaging tests — advanced lipid, metabolic, and inflammatory panels, full-body biomarker testing, or a coronary CT angiogram (CCTA) — through the traditional system. It usually comes down to two things:

No — we complement your doctors, we don't replace them. 3 Billion Beats makes you a more informed, empowered participant in your own care. Your membership includes 99+ lab biomarkers annually — roughly five times more than an average physical — plus access to advanced imaging, so you understand your health in far greater depth. You (and Corwin) can bring these insights to your PCP or cardiologist to make your visits more productive and personalized. Doctors may come and go, but 3 Billion Beats is built to stay with you.
Your physicians remain essential — they diagnose, treat, and manage your medical care. What we add is a complementary, functional-medicine lens: heart disease isn't a single problem but a multifactorial, systemic imbalance involving inflammation, oxidative stress, metabolic dysfunction, and lifestyle. All services are ordered and supervised by our affiliated physician practice, licensed in all 50 states. Our goal is health optimization and proactive care, not just management.
Why do I need these advanced tests if my cholesterol is "normal"? About 3BB

All clinical services — including lab ordering, CCTA imaging, and medical oversight — are provided by our affiliated physician-owned Professional Corporation (PC), whose supervising physician is licensed in all 50 U.S. states.
This MSO-PC structure is the same model used by many leading healthcare organizations. It ensures that clinical decisions are always made by licensed physicians while allowing us to deliver the best possible technology experience.
What plans are available, and what do they cost? Membership & Pricing
Just one — and that's the point. The 90 Nation is our single membership, named for a simple population statistic: 90% of heart attacks are preventable. Welcome to our family — everything is included:

Full refund within 7 days of your first payment. We want you to join because it feels like family, not because you're locked in. Try the membership, meet Corwin, see your assessment — and if the 90 Nation isn't right for you, we'll refund your first payment in full within the 7-day window.
Why do NY & NJ members pay a state lab fee? Membership & Pricing
Radical transparency is how we run this family, so here it is plainly: NY & NJ: +$371/yr state lab fee — passed through at our exact cost. Your state's lab regulations cost more to serve; we don't mark that up.
We don't mark up the labs anywhere — your annual panel is included at our cost, and membership pays for the care around it. The state fee works exactly the same way: what the extra regulation costs us is what you pay, to the dollar.
How often should I re-test? Membership & Pricing
Your 90 Nation membership includes a comprehensive panel every year — that annual rhythm is built into the membership. If significant interventions are made (dietary changes, supplements, etc.), your care team may suggest follow-up testing on key markers 3-6 months later to track progress.
Does 3 Billion Beats accept insurance? Insurance & HSA/FSA
It depends on the service — and we've designed it that way on purpose.
CCTA imaging: insurance supported. When you elect to use insurance for cardiac CT imaging, we verify your eligibility and coverage in real time, coordinate prior authorization when it's required, and facilitate claims through our affiliated physician practice. A transparent cash-pay path is always available too.
Labs: transparent self-pay, at our cost. Your annual comprehensive lab panel is included in your 90 Nation membership — we don't bill insurance for it, and we don't mark it up. Lab pricing through insurance is notoriously confusing: the same test can carry wildly different "list prices," and many plans deem preventive tests "not medically necessary" until after a cardiac event has already happened. Going self-pay lets us sidestep all of that and order the most comprehensive advanced panels — markers like ApoB, Lp(a), and inflammation panels — without those restrictions.
And because we make no money on the labs, there's nothing hidden: your annual panel is included at our exact cost, with no markup and no surprise bills. Membership pays for the care around it. We take care of you like we take care of our own families.
Can I pay with my HSA or FSA card? Insurance & HSA/FSA
Yes — you can use your HSA or FSA debit card directly at checkout, no reimbursement paperwork required. Lab testing and diagnostic imaging are typically HSA/FSA-eligible medical expenses, and we provide an itemized receipt you can submit to your plan administrator.
We also partner with Plaid, a secure financial platform, so you can check your available HSA/FSA balance — and put the money you've already set aside toward investing in your health.
How does insurance work for CCTA imaging? Insurance & HSA/FSA
When you elect to undergo a CCTA through our platform, we handle the insurance process for you:
- Eligibility Check: We verify your insurance coverage in real time using your insurance card information.
- Prior Authorization: If your plan requires it, we coordinate prior authorization through our affiliated physician.
- Facility Coordination: We schedule your scan at an in-network imaging facility near you.
- Cost Transparency: You'll see your estimated out-of-pocket costs (copay, coinsurance, deductible) before committing.
If you prefer not to use insurance, self-pay / cash-pay pricing is always available with transparent upfront costs.

We accept all major credit and debit cards and HSA/FSA debit cards. Prefer to spread it out? Membership can be paid over time with Affirm or Klarna (from ~$75/mo on the annual plan). For CCTA cardiac imaging, we also support insurance billing. All transactions are processed securely through PCI-compliant payment processors.
How does the testing process work? Testing & Results
Once you join the 90 Nation, our affiliated physician reviews and orders your annual comprehensive lab panel — included in your membership at our cost. You receive a requisition and visit a Quest Diagnostics location near you for a simple blood draw. Once the results are ready, they are securely uploaded to your 3 Billion Beats dashboard, where our AI analyzes the data and generates your personalized Health Story with actionable insights.
What key biomarkers are included? Testing & Results
Your included annual panel tests 99+ biomarkers — roughly five times more than an average physical. Key advanced markers include:

Do I need to fast before my blood draw? Testing & Results
Yes. For the most accurate results, particularly for lipid and metabolic panels (like insulin and triglycerides), we require you to fast for 10-12 hours before your blood draw. Water and black coffee are generally acceptable.
Where do I go for my blood draw? Testing & Results
All lab work is performed at Quest Diagnostics locations nationwide. With thousands of patient service centers across the U.S., there is likely a location convenient to you. After your physician-supervised requisition is generated, you can walk into any Quest location — no appointment needed in most cases.
What is a CCTA and why would I need one? Testing & Results
A Coronary CT Angiography (CCTA) is a non-invasive cardiac imaging scan that provides a detailed 3D view of your coronary arteries. Unlike stress tests, which typically detect blockages only after they reach 70%+, a CCTA can identify plaque buildup at much earlier stages — allowing for intervention before a cardiac event occurs.
We coordinate the entire process: physician ordering, insurance verification, facility scheduling, and AI-powered plaque analysis through our Cleerly AI integration.
Is a CCTA covered by insurance? Testing & Results
In many cases, yes. CCTA is a well-established diagnostic imaging procedure (CPT 75574) and is covered by most major insurance plans when ordered by a physician with appropriate clinical indication. Our platform verifies your coverage in real time and provides cost estimates before you commit.
A self-pay / cash-pay option is always available if you prefer not to use insurance.
